Registration Requirements
Teen Summer Registration is Now Open!
All teens must meet the following requirements to register for NORD Teen Summer Camp. All documentation must be current for 2026.
1. Proof of Income (CURRENT YEAR: 2026)
Provide documentation showing household income:
- SSI Award Letter with monthly amount
- Food Stamp or Social Security Award Letter with amount
- Four consecutive pay stubs for all adult household members
- If unemployed: Letter from Louisiana Workforce Commission or notarized statement of income
- If self-employed: Notarized statement of gross monthly income
- Two-parent households: Documentation for both parents/guardians
2. Proof of Residency (Orleans Parish): (CURRENT YEAR: 2026)
Provide one document showing current Orleans Parish address:
- Utility bill, phone bill, or insurance statement
- Pay stub
Address must match the one on the registration application.
3. Identification Verification
- Photocopy of a valid ID for parent(s)/guardian(s)
- If there are 2 parents/guardians, provide documentation for both
- Teens without ID: report card fulfills this requirement
4. Report Card or Immunization Record
- Teens enrolled in school: Current 2025-2026 report card
- Teens not yet in school or homeschooled: Up-to-date immunization record
5. Teen’s Birth Certificate
6. Teen’s Social Security Card

Important Notes
- All required documentation must be submitted at the time of registration. Incomplete applications will not be accepted.
- If registering multiple teens from the same household, provide copies of all documents for each teen.
- NORD will not make copies on-site. Bring originals for verification.
